the questionnaire

the goal of this questionnaire is to understand the needs of queer folx, available resources, and how faith affects and influences our identities. the term queer is used here as a general term for the lgbtqia+ community. i understand that not everyone will identify with this particular descriptor, and there will be an opportunity to indicate one's gender identity, expression, and sexuality on the form.

note: this questionnaire will take some time and could be taxing, if not triggering. please take care of yourself first and foremost. aspects of responses may be published online or in future print publications where appropriate, but any sensitive identifying information will be withheld as necessary for your privacy and protection.

some questions may not be applicable. please only answer the ones you feel resonate and relate to you the most (even if they are starred: just write me a note and move to the next one).
